Association of Ontario Health Centres
The Association of Ontario Health Centres (AOHC) is “Ontario’s voice for community-governed primary health care”, representing over 120 community-governed primary health care organizations. These include Ontario’s Community Health Centres, Aboriginal Health Access Centres, and Community Family Health Teams. A community board made up of community members, clients, health providers, and community leaders governs each Centre.
- The AOHC website includes a number of resources and tools about the Community Health Centre Model of Care, including a 2009 manual entitled Ontario Community Health Centres: Our Model of Care. This manual details a model of care that is comprehensive, accessible, client- and community-centred, inter-professional, integrated, community governed, strives to improve the social determinants of health, and is grounded in a community development approach.
