Health Nexus (formerly the Ontario Prevention Clearinghouse) is a registered charitable organization that supports the development and implementation of prevention and health promotion strategies that aim to enhance well-being and reduce health inequities.

Health Nexus is a partner with the Health Equity Council in Building Capacity for Equity in Health Promotion, a collaborative and multi-site project focused on addressing inequities in health experienced by racialized communities, especially in low income communities. A literature and resource guide are available.
In partnership with the Ontario Chronic Disease Prevention Alliance, and the Canadian Cancer Society – Ontario Division, Health Nexus published A Primer to Action – Social Determinants of Health, which explores how social determinants impact chronic disease.
